You are entitled to the dental benefit membership program for 12 consecutive months after the plan is paid in full. This is an annual reduced-fee wellness plan that allows individuals to receive quality dental care from Maplewood Dental Associates for a percentage off the office’s usual, customary, and reasonable (UCR) fees. This is not an insurance plan and no claims need to be filed.
The 12-month membership fee is due in full upon joining. Membership is effective on the day on which payment is received. We will help you schedule appointments; however, if appointments are not used, the member is not entitled to a refund. Membership rates may be reviewed and adjusted annually, with prior notification.

● Some dental work may be referred to specialists if deemed too complex.
